POLICY CHANGE – January 1, 2011



Effective January 1 2011, Pediatric Associates of Fort Collins will be implementing new policies,
regarding patients arriving late for appointments and cancellation notices in order to provide
efficient patient care.

Your Visit
In order to expedite your visit, it is imperative that you arrive at least 5 minutes before your
appointment time. Please also be prepared with the following:

-        Current Insurance Card – In order for us to properly bill your visit, we need to have
current insurance information on file. Failure to have current information delays payment on claims.
-        Copays need to be paid at time of service, when checking in. This will allow the check-out
process to move along quickly. We accept checks, cash, Visa, Mastercard and Discover.
-        If you do not have insurance, today’s services must be paid in full.
-        If you have a previous balance, payment will need to be paid in full or ask to speak with
Christy for other arrangements.

Arriving On Time for Appointments
Being on time for appointments is very important. If one person is late for their appointment, this
will delay appointments for all patients later in the schedule. If you are late we may need to re-
schedule you for another time. A charge for a missed appointment will be incurred.

Cancellation/Late Policy
Our office is requiring at least 24 hours notice for cancellation of any well care or medication
follow-ups. We also require 2 hours prior notice for cancellation of any other appointment. If we
are not given the appropriate cancellation notice or an appointment is missed, the charge will be
$50.00 for well care and $25.00 for all other visits. These charges will not be billed to your
insurance. If an appointment is missed 3 times you will be asked to transfer care to another
practice. To cancel an appointment, please call (970) 484-4871.


When making your appointment if you have multiple concerns and feel that you may need more time
than we normally allow please let the person scheduling the appointment know, so we can give you
the attention you need.
